Privacy policy

The Chekhter Center for Traditional Chinese Medicine (“TCC-TCM”, “we”) collects and uses personal health information in the course of providing care. In Ontario this is governed by the Personal Health Information Protection Act, 2004 (PHIPA). This page explains what we collect, why, and what you can ask us to do about it.
Who is responsible
Katherine Chekhter, R.Ac., R.TCMP, is the health information custodian and the contact for any question or complaint about your information. Requests to see, copy or correct your record, and any withdrawal of consent, should be made in writing to the clinic address below, which is also our mailing address.

What we collect
Through this website
- Nothing is collected automatically for marketing. This site sets no advertising or tracking cookies and does not use Google Analytics.
- Our host records standard server logs, including IP address and browser type, for security and reliability. These are not used to identify you.
- Fonts are loaded from Google Fonts, which means your browser contacts Google’s servers when you open a page.
When you book or attend an appointment
- Contact details: name, phone number, email address.
- Health information: your intake questionnaire, health history, assessment findings, treatment notes and any correspondence about your care.
- Insurance and payment details needed to issue a receipt.
Booking and clinical records are held in Jane, a Canadian practice management system, not on this website. Jane’s own privacy terms apply to that data alongside ours.
Consent
When you come to us for treatment we rely on your implied consent to collect, use and share your personal health information with other practitioners involved in your care. This is what PHIPA calls the circle of care.
Anything outside that needs your express consent: sending information to an insurer, to an employer, to a lawyer, or to a family member.
You can withdraw or limit your consent at any time by telling us in writing. Withdrawing may mean we cannot provide certain treatment, and we will say so if that is the case. Withdrawal does not reach backwards to disclosures already made.
When the law overrides consent
A small number of situations require or permit disclosure without your consent: where there is a significant risk of serious bodily harm, where a court order or summons requires it, where suspected child abuse must be reported, and in response to a lawful investigation by our regulatory College.
If something goes wrong
If your personal health information is lost, stolen, or seen by someone who should not have seen it, we will tell you at the first reasonable opportunity and notify the Information and Privacy Commissioner of Ontario where the law requires it.
Why we collect it
- To assess, plan and provide treatment.
- To keep the clinical records that our regulatory College requires.
- To contact you about appointments.
- To issue receipts you can submit to your insurer.
We do not sell your information, and we do not use it for advertising.
When we share it
We share your personal health information only with your consent, or where the law requires or permits it — for example, a legal obligation to report, or a College investigation. We do not submit claims to your insurer on your behalf unless you have asked us to and agreed to it in writing.
How long we keep it
Clinical records are kept for at least ten years after your last visit, as the College of Traditional Chinese Medicine Practitioners and Acupuncturists of Ontario requires.
Where the patient was under 18 at the time of treatment, the record is kept for at least ten years from the day they reached, or would have reached, the age of 18. We keep any record longer where the law demands it, for example while a claim or an investigation is open. Once a record passes the end of its retention period it is securely destroyed: paper is shredded, and electronic records are deleted from Jane and from any backup that holds them.
How it is protected
Records are held in access-controlled systems. Paper records, where they exist, are stored securely at the clinic. Access is limited to those who need it to provide your care.
Your rights
- You may ask to see your record and to receive a copy.
- You may ask us to correct information you believe is wrong.
- You may withdraw consent for particular uses, though this may affect the care we can provide.
- You may raise a concern with us at any time.

Changes to this policy
If this policy changes, the revised version will be posted here with a new date at the top.
